Phoenix House of Venice is currently hiring or a Care Coordinator. The Care Coordinator is responsible for the clinical functions of determining resource needs of a caseload of clients and their families and facilitating client and family?s enrollment in medically necessary services before, during, and after discharge from substance abuse treatment. The Care Coordinator will assist clients and their families in beginning and sustaining their enrollment in substance abuse treatment, he/she will be responsible for individual service coordination for Phoenix House clients in the program, and will ensure client/family access to needed services upon discharge. The Care Coordinator will provide case management services, conduct assessments and screenings, will advocate for client and client?s family, and will be available for consultation with the service partners via teleconference, phone, and in person. The Care Coordinator will be responsible for attending case conferences and facilitating connections to resourced determined necessary to the client?s treatment and ongoing recovery. The Care Coordinator will also work with the client?s primary counselor/therapist to oversee the integration of other services, such as housing, legal, educational, and vocational services into the client?s treatment episode. The Care Coordinator will be responsible for developing and maintaining relationships with service partners and maintaining a resource database to facilitate linkage between clients and services in their communities.
Bachelor?s degree required in related field; BSW preferred
Minimum of 2 years of relevant experience; in a case management setting serving under-resourced populations
oAt least 1 of these years preferred to be serving primarily clients with substance use or mental health disorders
1+ year of prior experience with an electronic health record preferred
Assists clients in obtaining health insurance through any and all eligible means, if not already covered.
Facilitates client and family education regarding health conditions/diagnoses, including making self available for parent questions and concerns.
Supports care management team by recording contacts with referral partners in a coordinated database.
Assists treatment team in finding needed resources for clients and families
Coordinates continuity of patient care with patients and families following admission and discharge, including creating and maintaining a case management (transition) plan.
Assists support staff in daily patient interactions as needed and as available.
Promotes clear communication amongst a care team and treating clinicians by ensuring awareness regarding patient care plans.
Participates on a team for data collection, health outcomes reporting, clinical audits, and programmatic evaluation related to Managed Care Initiatives.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
